I went through this myself and couldn't believe Shopify didn't support this, so after much searching I used Kite - https://apps.shopify.com/kite-free-gift - I had to avail myself of the Customer Support multiple times to get the exact functionality I wanted, so if you are having any trouble executing on what you want reach out to their team and they'll custom build it for you for free. let me know if you have any questions!
